Regeneron's new Phase II COURAGE disclosure suggests trevogrumab can reduce lean-mass loss when added to semaglutide. In the lower-dose portion of the study, the 75 mg combination preserved 42.5% of the lean mass otherwise lost at 52 weeks, relative to semaglutide with placebo, using DXA measurements.
A predefined MRI substudy reported approximately 70% preservation of thigh muscle volume. These are different measurements and should not be presented as interchangeable results. Regeneron said the lower-dose combination did not meaningfully increase total weight loss.
The company plans another Phase II study in older adults with obesity and reduced muscle mass or strength. That population could help test whether preserving tissue translates into outcomes patients can feel and use.
Commercially, this proposes an additional dimension to obesity treatment. A medicine need not maximise kilograms lost to offer value. However, body-composition changes alone do not establish improved mobility, independence or fewer falls. Those functional questions are important to any eventual case for adding another therapy to treatment.
