GSK announced US accelerated approval of Jemperli for adults with stage II or III locally advanced rectal cancer whose tumours are microsatellite instability-high or mismatch repair deficient. The decision creates an immunotherapy option that could help eligible patients avoid chemotherapy, radiation and surgery.

The supporting AZUR-1 study requires careful interpretation. Independent central review found sustained clinical complete responses at 12 months in 47% of 116 evaluable patients. Investigator assessment produced a higher figure, 68% of 144 patients. These are different assessments with different denominators, not interchangeable estimates or proof that every patient is cured.

For GSK, the approval extends the commercial role of an established oncology medicine into a precisely defined population. Biomarker testing and specialist treatment pathways will influence uptake alongside the clinical evidence.

Accelerated approval also leaves an obligation to confirm benefit. The commercial opportunity is meaningful, but the label applies to a subset of rectal cancers. Neither the early-study publicity nor the new authorisation supports extrapolating these results to all colorectal cancer patients.
